3. Diploma Certificate (graduated within 5 years) or Degree Certificate (graduated within 1 year) – in a Technical/Engineering field.
4. Copy of owner’s or director’s identification card.
5. Owner’s company mobile phone number.

1. Audited Accounts/Statement of Account/Profit and Loss Statement/Balance Sheet for the last 3 years, for example: 2021, 2020, and 2019.
2. Letter of Appreciation (6 LA/PO).
3. CIDB Course Certificate – with CIDB logo – (2 certificates, for example, CCD, Safety Course, SKP, SKK).
4. EPF/SOCSO.

2. Copy of owner’s, director’s, or employee’s birth certificate.
3. Attachment 1 – Work Declaration (to be filled out by all individuals in the CIMS system).
4. Attachment 2 – Bank Confirmation Letter (to be filled out by the individual signing the check/bank transaction).
5. EPF Form A + payment receipt if contributing for employees.
6. Sketch of the premises location (such as a marriage certificate) – (Google Maps cannot be used).
